Purpose of use of Personal Information obtained in direct writing

Personal information shall be obtained in a legal and fair manner, and be used within the scope of the purpose of use listed in the table below. In the event that a purpose of use not included in the scope of the “Purpose of use” arises, if the information has been obtained directly in writing, we will obtain the consent of the owner of the personal information regarding the new purpose of use, and if the information has been obtained by other means, publicize the new purpose of use before using the information for that purpose.

Purpose of Use of Personal Information Obtained Other than in Direct Writing

UC Technology will use personal information obtained other than in direct writing within the scope listed in the table below. When a purpose of use of personal information not included in the table arises, the information will be used after the purpose of use is publicly announced.

Security management measures

We take the following measures to prevent leakage, loss, or damage to personal information.

Organizational security management measures
  • Establish rules for handling personal information for each stage of acquisition, use, storage, provision, deletion, disposal, etc., with respect to handling methods, responsible persons, persons in charge, and their duties, etc.
  • Periodically inspect status of personal information handling
Human security management measures
  • Regularly provide training for employees on matters of relevance regarding the handling of personal information.
Physical security management measures
  • Implement access control for employees and establish restrictions on devices, etc. which are brought in areas where personal information is handled, as well as to take measures to prevent unauthorized persons from viewing personal information.
  • Take measures to prevent theft or loss of devices, electronic media, documents, etc. that handle personal information, and protect such devices and electronic media by password protection or data encryption, when they are carried inside or outside company premises.
Technical security management measures
  • Implement access control to allow only the authorized persons to handle the personal information, and to limit the scope of personal information database handled by such authorized persons.
  • Introduce a mechanism to protect the information system that handles personal information from unauthorized access from the outside, or from malware.
Understanding overseas environments
  • Implement safety management measures after understanding the system regarding the protection of personal information in foreign countries where personal information is stored.

Requests for Disclosure of Personal Information

  1. When an individual requests the notification of purpose of use, disclosure, correction, addition, deletion of content, suspension of use, deletion, or the suspension of provision to a third party (hereinafter “disclosure, etc.”), UC Technology will internally review the personal information and will respond without delays.
    * We may not be able to accept requests for disclosure, etc. in certain instances depending on the content of the request. In such cases, we will inform the individual of the reason(s).
  2. To request disclosure, etc., please mail us the required documents following the correct procedure. Incomplete documents may not be accepted. For each request of disclosure, etc. of personal information or notification of purpose of use, an administrative fee of 1,080 yen will be charged.
  3. Personal information received on the “Personal Information Disclosure Request Form” will be used to respond to inquiries and will not be used for any other purpose.

To request disclosure, etc., please send the required documents by recorded mail so that the delivery can be tracked and traced.

Request via Proxy

When a legal representative or representative delegated by a principal requests disclosure, etc., for the principal, please enclose the required documents below in addition to the documents mentioned in the preceding paragraph.

1. Document to Prove the Legal Representation Right for the Principal

  • One transcript of the family register certificate. In the case of a representative with parental authority, one copy of the health insurance card with the dependent’s information is also acceptable.

  • One copy of a document that can verify the name and current address of the designated representative, such as a driver’s license, passport, or health insurance card

1. Power of Attorney Form (to be attached to “Personal Information Disclosure Request Form” for the request)
